I agree to exempt, waive and relieve Guide Dogs SA/NT from liability for personal injury, property damage, or other claims that may result from the client undertaking Boss’ Blind Date, and any activities under blindfold. I agree to not take stairs while blindfolded and will remove my blindfold where stairs cannot be avoided.

I/we comprehend and appreciate that there are foreseeable, unforeseeable and inherent dangers and risks of harm involved in undertaking activities while blindfolded. I/we acknowledge that the activities can be physically and mentally intense, and waive Guide Dogs SA/NT from liability.

I give permission for Guide Dogs SA/NT to use without fee or payment the whole or any portion of my image in promotional materials for Guide Dogs SA/NT or any future promotion from the date entered below and for as long as they choose.

Guide Dogs SA/NT may use the image on digital communications platforms, social media channels or promotional materials (printed or digital). The image may also be provided to media outlets for fundraising purposes or for promoting the work of Guide Dogs SA/NT.

I acknowledge that I may request removal of my image from databases at any time by notifying the Guide Dogs SA/NT Marketing and Community Engagement Coordinator in writing.

I have read and understand the rules, terms and conditions of participation in Boss' Blind Date Activities and will comply with them at all times.
